Equipment Loan Agreement

The London Blizzard Sledge Hockey Club is pleased to provide equipment to athletes who require access to para ice hockey equipment. This equipment is provided to help athletes participate in the sport and remains the property of the London Blizzard Sledge Hockey Club.

Equipment Being Loaned

Conditions of Loan

1. All equipment remains the property of the London Blizzard Sledge Hockey Club.

2. Equipment is provided solely for participation in para ice hockey activities.

3. The athlete is responsible for reasonable care and maintenance of the equipment while it is in their possession.

4. Equipment may not be sold, transferred, loaned to another individual, or modified without approval from the London Blizzard.

5. Any loss, theft, or significant damage must be reported to the Club as soon as possible.

6. Normal wear and tear is expected and will not result in penalties.

7. Equipment must be returned promptly upon request by the Club or when the athlete is no longer participating with the London Blizzard.

8. Failure to return equipment may affect future participation and equipment loan privileges.

Agreement

I acknowledge receipt of the equipment listed above and agree to the terms and conditions outlined in this Equipment Loan Agreement.
