London Blizzard Sledge Hockey Club is committed to ensuring that financial circumstances do not prevent athletes from participating in para ice hockey. Families and players requiring additional time to pay registration fees may request a payment plan through this form.

All payment plan requests are kept confidential and reviewed with the understanding that every situation is unique.
